Exhibitor Registration

Cancellation Policy: Cancellations and requests for refunds must be submitted in writing to the MOA prior to September 12, 2012.Booth Specifications: Each 10′ x 10′ booth will have 8′ back and 3′ sides and will include pipe & drape, one 6′ skirted table, two chairs, one waste receptacle and one 7″ x 44″ identification sign.

Note: You will be receiving an Exhibitor’s Packet from Heritage Exposition Services approximately 8 weeks prior to the show providing request forms for additional equipment (i.e. electricity, extra tables, shipping information, etc.) . Contact Heritage Exposition Services directly with any questions for booth set-up.

Booth assignments will be made on a first-come, first-served basis. Every effort will be made to avoid placing similar labs and/or suppliers next to each other. Booth registration fee includes admission for two to the MOA Awards Luncheon. Please RSVP to MOA’s Central office, the number of reps expected to be in attendance at the luncheon.

Note: To reserve booth space online, you must use a credit card. Once you have entered your credit card information and completed the transaction, you will receive an email confirmation. If you DO NOT receive an email confirmation, please contact Sue (573-635-6151) at the MOA Central Office to make sure your registration was completed.

Note: Exhibitors, please enter names of all attendees on the next page during the check-out process. By clicking the “Buy Now” button the exhibitor assumes the entire responsibility and liability for losses, damages, and claims arriving out of injury or damage to exhibitor’s displays, equipment, and other property brought onto the premises of the hotel and shall indemnify and hold harmless the hotel agents, servants, employees, and Missouri Optometric Association members and Board of Directors from any and all such losses damages, and claims.
